Summary: The Clinical Integration Coordinator has responsibility and accountability for evaluating, facilitating improvements in, and reporting on the quality of clinical care delivery within Northeast Medical Group. This position will report to the Executive Director, Clinical Integration but will be an embedded member of the Northeast Medical Group team, working closely with Medical Directors and the CEO of Northeast Medical Group.

The Clinical Integration Coordinator will support the Vision of Northeast Medical Group to deliver the highest quality, accessible health care to patients and support clinical integration across the Yale New Haven Health System. Quality clinical care is defined by the Institute of Medicine as care that is safe, effective, patient-centered, timely, efficient and equitable. The Clinical Integration Coordinator must be a dynamic individual with a passion for improving the quality of health care delivery in the ambulatory setting and across the continuum of care. Working with management, staff and clinicians, the Clinical Integration Coordinator: supports NEMG practices and specialty practice teams to reduce variation and standardize clinical care and operational effectiveness according to best practices; supports NEMG practices in efforts to build clinical effectiveness, become more clinically integrated and improve the patient experience; and assists NEMG and the member practices to collect, document and report relevant patient safety and clinical quality metrics.

Special Skills: A passion for improving the delivery of health care in the ambulatory setting and across the continuum of care . Superior communication skills, including presentations. An ability to work collaboratively with and influence individuals at varying levels within the organization. Past experience delivering results in a matrixed work environment.

Education: Master’s degree in business, public health, nursing or other related field strongly preferred. Valid CT RN license strongly preferred. Bachelorís degree required. Advanced training in performance improvement methodologies including LEAN is preferred.

Experience: At least five (5) years of experience working within a health care setting with progressively increasing responsibilities. Experience in data analysis, statistics and performance improvement is required.
